Previous experience providing pharmacotherapy specialist services, including comprehensive medication management, optimization of complex drug regimens, therapeutic drug monitoring, management of high-risk or chronic conditions (e.g., cardiology, infectious diseases, oncology, critical care), prevention and resolution of adverse drug events, interdisciplinary team collaboration, patient counseling, and evidence-based interventions to improve medication safety, efficacy, and therapeutic outcomes across diverse patient populations (in-person or via telehealth where appropriate).
